Important Announcement for HACC clients
Home and Community Care (HACC) Funding is now known as CHSP
From 1 July 2018, Home and Community Care (HACC) services for older people in Western Australia, will be funded and managed by the Department of Health through the Commonwealth Home Support Programme (CHSP).
If you are currently a Southern Plus client receiving HACC services including domestic services or respite services in one of our respite centres, you will not need to do anything. Our clients will continue to receive the same amount of funding from CHSP and the same services including our same face promise from a dedicated wellness partner.
CHSP provides entry-level home support and respite for older people aged 65 years and over, and 50 years and over for Aboriginal and Torres Strait Islander people who need assistance to keep living independently at home and in their community. A home support assessment conducted by the Regional Assessment Service (RAS) is required to access Government subsidised support at home and we can help you arrange access to these services through My Aged Care.
